Secondary prevention of lower limb sport injuries and its application to the tactical athlete: a systematic review of systematic reviews

Abstract
Aim: The purpose of this systematic review of systematic reviews is to mitigate tactical athletes’ lower limb injuries and reinjury rate as well as optimising their recovery time for return to duty. To this end, a synthesis of findings regarding secondary prevention related to athletes has been assessed in order to apply them to tactical athletes. Method: A systematic review of systematic reviews has been realised following the Cochrane overview recommendations. 5 databases were sought (PubMed, Cochrane, Scopus, SPORTDiscus, EMBASE). Systematic reviews (SR) written in English or French, published over the last 5 years and answering to the PICO’s criteria were included. Results: A total of 9 SR were yielded. The main results that could be applied to tactical athletes were: (1) loading the muscle during extensive lengthening leads to a shorter rehabilitation period, and 4 daily static stretching sessions would be better than 1; (2) eccentric exercises reduce strain injury risk but the alleviation of pain during isometric contraction is a priority before implementing concentric and eccentric ones; (3) the ability to sprint and perform military training movements without pain may indicate readiness to RTD; (4) recurrences of the injury increase risks of reinjury; (5) objective measures of isokinetic dynamometry added with the implementation of range of motion tests may provide a good guide to clinical recovery and point out a risk of reinjury. Conclusion: The main aspects to consider for the tactical athletes’ rehabilitation are strength, mobility and recurrences of the injury. Besides, it is important to consider the secondary prevention in a holistic way. Nevertheless, due to the heterogeneity of the results and the lack of diversity of the injuries assessed, it would be interesting to pursue other researches in order to have a more general applicability.
