- This project presents several tools that promote the exchange of information between patients and practitioners. The first tool is a desktop application that allows health practitioners to create a small electronic health record (EHR) and share it with other practitioners. It also allows them to send essential data to their patients with chronic diseases via a RESTful web server. The website part of the project allows the patients to contribute to their EHR and add their own data. The project only targets the diabetic patients and those suffering from heart failure but offers the possibility to extend it to other chronic diseases.
