Abstract
Abstract: Background: Emerging intracardiac three-dimension (3D) navigation systems (for ex. Ensite NavXTM, Abbott) have become popular for catheter tracking during the treatment of many arrythmias. Despite the significant reduction of ionizing radiation exposure compared with standard fluoroscopy (RX), patients and the personnel working in the Electrophysiology (EP) Lab are still at risk of developing radiation associated injuries. Objective: Our aim was localizing hotspots spatial patterns of radiations exposure for patients and operators during RF ablation of typical Atrial Flutter (AFL) using 3D and RX catheter tracking. Methods: Patients diagnosed with typical AFL and undergoing RF ablation of the cavo tricuspid isthmus (CTI) were randomized either to 3D or RX groups. To monitor exposure to radiation during the ablation, 8 thermoluminescent dosimeters (TLDs) were used. Five were placed on the patient: 2 in anterior (front head -FH, thyroid -T) and 3 in posterior positions (thorax left -L, left paravertebral -LP, and medial -M). Three were pinned to the operator: front head -FHo, thyroid -To, and left hand -LHo. Procedural parameters such as Rx time -tRx, dose area product-DAP, procedure time -tEP were recorded. Results: Eighteen patients mean age 64 years, with a male prevalence (73%), were randomized. Patients from the Rx group shows significantly increased RXt (540 [IQR 238; 948] mGycm2) vs (150 [IQR 83.0; 262] mGycm2] (P, 0.017), DAP (4755 [IQR 1949; 5700] mGycm2) vs (783 [IQR 454;2507] mGycm2) (P, 0.013), T (0.50 [IQR 0.38; 0.80] mGycm2) vs (0.32 [IQR 0.24; 0.48] mGycm2) (P=0.043), LP (1.03 [IQR 0.71; 1.19] mGycm2) vs (0.36 [IQR 0.24; 0.80] mGycm2) (P=0.05) & M (1.86 [IQR 1.34; 2.43] mGycm2) vs (0.72 [IQR 0.28; 0.89] mGycm2) (P=0.009) values. Spearman correlation analyses between DAP and TLD data, shows that LP is the most associated variable with DAP (spearman rho=0.8, p<0.05). T values are significantly increased in Rx group and associate with To (spearman rho=0.73, p<0.05). Finally, FH, FHo & LHo values significantly correlate among them (spearman rho<0.05). Conclusion: We evaluate the radiation exposure of different anatomical regions using 3D and Rx catheter tracking during treatment of typical AFL. Recorded PSD and tRx values are aligned with the literature. TLD data, show that significant more radiation was received in the T, M and LA positions by patients in the Rx group. Results also suggest that higher thyroid radiation in patients correlate to high thyroid radiation in the operator. The significant positive association among FH, FHo & LHo values define an area where additional shielding, like radiopaque material (removable protections), should be implemented to increase safety for both patients and personnel.
