- Abstract
- Introduction: Human papillomavirus (HPV) is the most common sexually transmitted infection (STI) worldwide. Despite recommendations from health authorities, concerns were raised regarding vaccine uptake and whether vaccination could induce risk compensation in young women. The primary aim of this study was to investigate if HPV vaccination in young women in Luxembourg was associated with risk compensation of sexual behavior. Methods: Seven hundred twenty-nine women (mean age = 22.5 years) were recruited either at Luxembourg family planning centers or at private gynaecology practices. Participants completed a questionnaire on vaccination status and sexual behavior. Multivariable Poisson regression and logistic regression were used to study the association between sexual behavior and vaccination status. Assortativity of sexual mixing by age was also assessed for further transmission modeling. Results: In total, 71.8% of participants reported receiving the HPV vaccine. Vaccine uptake varied significantly by nationality and was higher in Portuguese (78.9%) and in Luxembourgish (71.6%) women. HPV vaccination did not induce riskier sexual behavior such as earlier age of sexual debut or increased number of sexual partners. The number of sexual partners was significantly lower in vaccinated than in unvaccinated women due to age difference between cohorts. Condom use, age at first intercourse, age difference between sexual partners and duration of relationship were not significantly different between groups. Assortativity analysis revealed that younger women tended to choose sexual partners who were on average 2 years older. Discussion: Our study found no evidence of an association between at increased risk sexual behavior and HPV vaccination, a reassuring finding.
